That this is still the case is truly a tragedy in 2011- physicians treating breast cancer patients must always remember that the deformity of breast cancer management can be incredibly devastating to patients. We must do better to restore our patients to COMPLETE health- including their sense of feminine beauty.

Sad, tragic, frustrating, wrong as it may seem, there can be no doubt that financial pressures have been causing detrimental changes in the way healthcare is delivered for some time now. We must all realize that as long as we continue to undervalue physician’s work product and diminish their compensation, the quality of care and experiences we have when in need will continue to decline. It is not reasonable to allow this to continue but be surprised and upset when we are personally affected by it.

There can be no doubt that lack of patient compliance and diligence with instructions and restrictions is a significant problem in the United States, contributing to less than ideal outcomes, rising costs, and even the medico-legal crisis we have been in. Anything that would help patient populations adhere to physician’s prescribed management plans ought to be taken seriously. The question will then be whether the added expenses will be acceptable compared to the diminished expenses of non-compliance.

This is an excellent article, and I agree with Dr. Verghese on the whole. On the other hand, he doesn’t speak of the fact (of which I am sure he is aware) that most American physicians feel compelled to order tests and that physical examination is less valuable because of the extreme lawsuit environment that our country’s doctors practice in. It’s difficult to rely on exam skills (which may miss a small percentage of findings even in the hands of the most skilled clinician) when you know that a CT scan will almost always make the right diagnosis and that the cost of you missing a finding may be your reputation and livelihood.

Unfortunately, the medical liability crisis and the issues Dr. Verghese raises are inextricably linked.

As with all health care options, it is important to realize that research showing patient benefits and what is in the best interests of your surgeon are sometimes at odds…

For this reason, I think you are most likely to get the very best advice by consulting with surgeons who are able to offer BOTH options- hopefully minimizing any incentive they may have to guide you towards one option or another.
